This page contains the documents and reference tools students use
throughout their field placement year. Download the forms you need
below, and refer to the
Field Education Resources and Information page for the Field
Manual and Field Calendar.
Learning Agreement
The Learning Agreement is a required document completed at the
start of your field placement. It outlines your individual learning
goals, the competencies you will develop, and the activities your
agency will provide to support your growth. Review the instructions
before meeting with your field instructor to complete the agreement.
Process Recording
Process recordings are a structured reflection tool used throughout
field placement. They help students analyze their interactions with
clients by documenting what was said, what was felt, and what
social work skills were applied. Your field instructor and faculty
field liaison will review your process recordings as part of your
ongoing supervision.
